Membership fees pay for enhanced access and non-covered amenities and services. They are not insurance, not a substitute for insurance, and not prepayment for any covered medical service. Members are strongly encouraged to maintain separate health insurance.
All memberships are cash-pay. No insurance is billed for membership or for services rendered under membership. Superbills are provided on request. Membership is terminable by the member on written notice with a pro-rata refund of unearned prepaid fees.
